During November and December, all residents of Fukuoka city have been sent a paper notification card bearing a 12-digit Individual Number. This “My Number” identification system will go into use starting in January.

Those who apply are able to receive an additional plastic “Individual Number Card” from January. Your individual number card will not only be used for a variety of city government paperwork, but also bears your photo and serves as a form of identification. You can use it for purposes such as issuing resident records at convenience stores and filing tax over the Internet.

Those who wish to apply for the card bearing their individual number can use the application form enclosed in the notification mails that have been sent out since November. Affix your photo to the form, sign it and return it using the special enclosed envelope. Applications are also accepted via smart phones and the Internet. For further information, please reference the pamphlet enclosed in the envelope with your notification card. It may take a month or longer to receive your card after your application is received. As soon as it is ready, you will be notified by your local ward or branch office.

From January of next year, your Individual Number will be required for procedures related to social security services such as welfare and healthcare, and taxation. In order to prevent abuse of the My Number system in administrative procedures, individuals identity will be verified. Paperwork can be completed using the Individual Number Card by itself, but passports, residence cards or other forms of personal identification are required if using the mailed notification card alone.
